owensboro-healthOWENSBORO, Ky, (6/29/14) — The Green River Partnership for Care Coordination, a collaboration between GRADD, Owensboro Health Regional Hospital and Henderson’s Methodist Hospital, was awarded a certificate of outstanding performance by Health Care Excel Kentucky last week.

Established in June 2013, the partnership has implemented the Care Transitions Program, through which GRADD “coaches” work with local Medicare patients to prevent frequent hospital readmissions.

CTP is designed to benefit clients, hospitals and taxpayers. Personalized follow-up consultations help clients remain healthy and in their own homes; hospitals avoid financial penalties (under the new Affordable Care Act) for excessive readmission rates; and taxpayers profit when waste within the Medicare system is eliminated.

“This program demonstrates the power of public-private cooperation,” said Bill Parrish, GRADD chairman. “It supports vulnerable residents in our region while also helping to control health care costs.” Health Care Excel Kentucky, based in Louisville, is a nonprofit consulting company that serves as the Commonwealth’s Medicare Quality Improvement Organization.
